Making a donation to LCSA

To help people make a donation to The Liver Cancer Surgery Appeal, we have produced a number of forms. These can be found below and each form is in Microsoft Word format. You may find it useful to download a copy of each form so you can decide which you feel best suits the type of donation you are interested in making.

The 'Basic Donation' form allows you to make a single donation.

The 'Gift Aid Declaration' form should be filled in and, together with a cheque, sent to the LCSA. As a registered charity the LCSA will be able to reclaim the tax on all donations from UK taxpayers.
